The change to phone number for a pet ownership record does not require a form. It can be changed over the phone, in person or email. Some may provide a letter or have completed the change of details form. All these can be accepted.

- Retrieve animal records. Go to 'Property and Rating' and search via animal id or address where the animal lives
- Ask the customer to confirm the details of ownership. Also confirm with they know some of the details of the animal, like the name of the pet, breed, colour, etc
- After you are satisfied you are speaking with the owner of the pet, make the changes to the owner's name ID. This found via the associates on the animal record (instructions)
- If they have provided a form or letter, stamp and scan to Records
